Aphantasia
Ask most people to picture an apple and they see one. Ask roughly 2-4% of the population and they see nothing. Not a faint apple. Not an effortful apple. Nothing. The phrase "picture this" was, for them, an idiom the rest of us appeared to be taking strangely literally.
Adam Zeman, a cognitive neurologist at Exeter, named the condition in 2015 after a patient referred to as MX lost voluntary visual imagery following a routine cardiac procedure. MX could still recognize faces, navigate, and dream in images. He simply could not summon a picture on demand. Zeman borrowed Aristotle's word phantasia and prefixed the alpha-privative. The naming did something the condition itself had not: it gave the 2-4% a way to find each other. Within a year of the BBC's coverage, Zeman's lab had heard from over 14,000 self-identified aphantasics.
What the evidence shows
The cleanest objective marker is the binocular rivalry paradigm. Show a participant a red-and-green pattern, then ask them to imagine a red one for several seconds, then show a rivalry display. Typical imagers' subsequent perception is biased toward red. Aphantasics show no such priming. This is not a self-report — it is a perceptual consequence the participant cannot game. Joel Pearson's lab at UNSW has run versions of this since 2008.
Self-report uses the Vividness of Visual Imagery Questionnaire (VVIQ), built by David Marks in 1973. Score 16 (minimum) and you likely have aphantasia. Score 75 (maximum) and you have what Zeman calls hyperphantasia: imagery as vivid as perception, present in perhaps 3% of people. The two tails are not symmetric mirror images of each other in personality or profession, but both are sizable populations hiding inside the assumption that everyone's mental life resembles one's own.
Aphantasia is not one thing. Some lose only voluntary imagery while dreaming remains visual. Some lose imagery across all senses (no inner voice, no remembered music, no imagined touch) — sometimes called total aphantasia. Some have intact spatial imagination (can mentally rotate objects, navigate familiar rooms) without any pictorial content. The dissociations suggest the brain runs at least two separable systems where common sense assumed one.
Where it shows up
- Animators, illustrators, architects who function professionally without visualizing. Glen Keane (Disney animator) and Ed Catmull (Pixar co-founder) have both said publicly they cannot voluntarily visualize. They draw from structural and conceptual knowledge, not from a mental picture.
- Autobiographical memory tends to be flatter and more semantic. Aphantasics often remember that their childhood bedroom had a blue rug rather than seeing the rug. Some studies (Dawes et al., 2020) find reduced episodic richness; the underlying mechanism is contested.
- Reduced imagery-induced fear conditioning. Wicken, Keogh & Pearson (2021) showed aphantasics have a blunted skin-conductance response to imagined threat scenarios. This has obvious implications for PTSD treatment, where imaginal exposure is a core technique.
- Over-representation in computational fields and under-representation in fiction writing, in survey data — but the surveys are self-selected and the effect is suggestive, not established.
What's contested
The deepest question is whether aphantasics lack the experience of imagery or the introspective access to imagery they are still computing. The behavioural evidence (rivalry priming, pupil constriction to imagined bright scenes) suggests the experience is genuinely absent or much reduced, not merely unreported. But "absence of experience" is a metaphysical claim that perception science is poorly equipped to settle. We are using third-person measurements to adjudicate a first-person question.
A second contest: is aphantasia a disorder, a variation, or a trade-off? Zeman has consistently framed it as variation, not pathology — most aphantasics report no functional impairment and many discover it only as adults. But the memory and emotional-imagery findings hint at real differences in domains the person may not have noticed were different. The trade-off hypothesis (less imagery, more abstract or verbal processing) is appealing and almost entirely unproven.
Third: what causes congenital aphantasia? The acquired form (like MX's) sometimes follows damage to networks including the fusiform and occipital cortex. The congenital form shows no consistent structural lesion. Twin studies suggest heritability but the genetics are unmapped. There is no known intervention to produce or remove imagery in adults, despite a small industry promising otherwise.
Why this has to do with other realms
Aphantasia is the strongest available evidence that introspection is unreliable about basic features of one's own mind. For most of human history, "everyone has a mind's eye" was assumed not because anyone checked but because no one had a procedure to check. The condition was hiding in plain sight for millennia because the question "do you actually see a picture?" sounds like a question about wording rather than fact. This is the same epistemic failure mode that shows up in concept qualia debates and in the concept hard problem of consciousness: we cannot read off the structure of experience from the inside, and we have been over-confident that we could.
It also reframes what creativity requires. The Pixar story is the load-bearing example. If person ed catmull built the studio that defined visual storytelling for a generation without being able to visualize, then "imagination" is not what most people thought it was. It is closer to a capacity to manipulate structure, of which visual rendering is one optional output.
An open question
If 2-4% of people lack voluntary visual imagery and most never notice, what other basic features of mental life are similarly variable and similarly invisible to introspection? The inner voice (anendophasia) is the next candidate under investigation, and early estimates suggest it is at least as common.
